Overview
VNS Health RN Field Supervisors conduct initial and ongoing in‑home supervisory visits to patients, ensuring high‑quality care delivery and compliance with clinical standards. The role includes patient assessment, care coordination, caregiver training, and effective communication with the interdisciplinary care team.

- Perform initial and ongoing supervisory field visits to patients' homes to provide assessment, training support, in‑home coordination, and/or crisis management.
- Convey significant changes in the patient's condition, emergency interventions, or care plan changes.
- Report all interventions to the clinical manager and document findings electronically.
- Collaborate with the primary care physician and clinical team for full patient assessment, gathering medical history and medication information.
- Supervise the initial home patient assessment to ensure the HHA follows VNS guidelines and regulations.
- Identify continued training needs of the Home Health Aide and document the information electronically.
- Deliver the start‑of‑care packet and provide an overall review with the patient/caregiver on the initial visit.
- Educate and review the paraprofessional plan of care with the Home Health Aide (HHA).
- Participate in quality assurance and education programs as requested by management.
- Participate in special projects and perform other duties as assigned.
License and current registration as a Registered Professional Nurse in the State of New York required.
CPR/BLS Certification required.
Education- Associate's degree in nursing from an approved program required.
- Bachelor's degree in nursing preferred.
- Minimum of two years’ clinical home care experience as Registered Nurse required.
- Experience in a licensed home care service agency or CHHA settings preferred.
USD $77,200.00 – $96,500.00 per year.
