About This Role
The Home Care Registered Nurse provides age‑specific care for patients using the nursing process, collaborating with healthcare teams to implement treatment plans and ensure patient safety.
Highlights- Location: Staten Island, NY
- Schedule: Full-Time
- Conduct patient interviews and assess health status across various systems.
- Determine coping mechanisms and identify cultural requirements for patient satisfaction.
- Develop individualized patient goals and coordinate care activities.
- Reassess patients and revise care plans as needed.
- Administer prescribed treatments and maintain accurate documentation.
- Collaborate with physicians on treatment regimens and patient care.
- Educate patients and families about medications and health conditions.
- Perform related duties as required.
- Graduate from an accredited School of Nursing
- Bachelor’s Degree in Nursing preferred; must enroll in a BSN program within 2 years and obtain a BSN within 5 years
- Experience in home care preferred
- Current License to practice as a Registered Professional Nurse in New York State required
- Specialized certifications as needed
