Overview

Position Summary:

Advanced Practice Provider Nurse Practitioner (Hemato‑Oncology, North Shore). The Nurse Practitioner uses clinical expertise to plan, implement, and evaluate safe, therapeutic, population‑specific patient care. The role involves working collaboratively within a multidisciplinary health team, ensuring care is patient‑centered and family inclusive.

• Performs other duties as assigned.
• Plans and organizes care to meet individual patient needs and ensures appropriate clinical resource utilization per protocols, pathways, and other means.
• Collaborates with the interdisciplinary team, including the attending physician, to develop and implement therapeutic plans and communicate with family members.
• Participates in the development and revision of interdisciplinary and nursing standards and patient/family education materials.
• Maintains timely communication with nursing staff, house staff, other disciplines, and ancillary services.
• Provides consultation to the nursing staff regarding the therapeutic regimen.
• Facilitates continuity of care across services such as home care, other facilities, and appointments with MDs.
• Demonstrates knowledge of organizational Services Standards and incorporates them into responsibilities.
• Participates in professional nursing standards and regulations of relevant outside agencies.
• Functions as a respectful member of the healthcare team.
• Performs complete health history, physical examination, and psychosocial assessment of patients within the designated practice or clinical unit.
• Orders laboratory, radiological, and diagnostic studies per protocols and follows protocols for abnormal findings, including referrals as appropriate.
• Formulates the plan of care with attending physician, patient, and family based on expected goals and length of stay.
• Collaborates with attending, consulting physicians, and other disciplines to maximize patient/family preferences and enhance quality, access, and cost‑effective outcomes.
• Prescribes inpatient and discharge medications, including controlled substances with DEA authorization, per New York State law.
• Prescribes medical equipment, devices, therapies, and home health services per practice protocol.
• Practices as a member of the allied health staff according to the rules and regulations of the medical staff and bylaws.
• Evaluates treatment and health care plans for effectiveness and modifies them per clinical standards.
• Considers needs and behaviors of specific patient age and cultural groups in all patient care.
• Promotes professional growth and development in clinical and managerial roles.
• Maintains current expertise in the area of practice.
• Serves as a clinical expert and resource for education of peers and other health professionals.
• Participates in the development and monitoring of patient outcomes per established protocols for quality improvement.
• Uses evidence‑based literature to advise and support practice changes within the designated service.
• Collaborates with nursing and other disciplines in the implementation of clinical studies.
• Assists in preparation, implementation, and evaluation of research protocols when applicable.
• Monitors and trends cost‑effectiveness of the position, particularly regarding efficiency and performance indicators.
• Manages Ostomy patients in the outpatient setting (Ostomy).

• Master's Degree graduate of an accredited Nurse Practitioner program.
• Current NYS Registered Nurse License.
• Current NYS Nurse Practitioner License.
• Current Nurse Practitioner Certification.
• Excellent oral and written communication, interpersonal, and organizational skills.
• Basic computer skills.
• Basic Life Support Certification through the American Heart Association.

• Registered Nurse License – NYS.
• Nurse Practitioner License – NYS.
• Basic Life Support Certification.

• Advanced Life Support certification (ACLS, PALS) through the American Heart Association or Neonatal Resuscitation certification (NRP) through the American Academy of Pediatrics (may be required based on selected privileges).
