Summary

Phoenix House is seeking a Licensed Practical Nurse to support clients’ health, safety, and recovery in a behavioral health and substance use treatment setting. In this role, you will help ensure clients receive attentive medical support and compassionate care throughout their treatment experience.

As part of a dedicated care team, you will monitor client health needs, support medication processes, document care, coordinate medical follow-up, and help create a safe, structured environment where clients can focus on healing and recovery.

Responsibilities
  • Provide direct nursing care to clients under the supervision and direction of clinical leadership.
  • Maintain patient confidentiality in accordance with Phoenix House policies and procedures.
  • Oversee unlicensed assistive personnel (e.g., CNAs, Medical assistants, and Medical Technicians).
  • Perform routine nursing procedures such as EKGs, wound care, and specimen collection.
  • Administer prescribed medications, including oral, topical, and injectable forms (IM, Sub‑Q, intradermal) in accordance with NYS scope of practice and facility policies.
  • Document vital signs, weight, and other clinical assessments to monitor patient condition and report significant findings to clinical leadership.
  • Observe and promptly report any changes in patient behavior, appearance, or health status to the appropriate clinical leadership.
  • Engage in health education and reinforce treatment plans, medication adherence, and self‑care strategies.
  • Other duties as assigned.
Licenses & Certifications
  • Current NYS Licensed Practical Nurse
  • First Aid Training
  • CPR Training
  • Narcan Training
  • Infection Control Certification
  • Child Abuse Certification
Experience
  • At least three years of related experience.
  • Qualified by training, education, and experience in providing nursing care for chemically addicted patients and patients withdrawing from alcohol and or drugs.
Competencies
  • Administration of subcutaneous and intramuscular injections
  • EKG proficiency
  • Proficiency in transcribing medication orders to the pharmacy and or the EMR
  • Proficiency of data collection of newly admitted clients
  • Obtain and document vital signs and escalating accordingly
  • Oversee the self‑administration of medications in a safe manner
  • Knowledge and adherence to universal precautions
  • Narcan administration
  • Computer proficiency
  • Customer service and satisfaction
  • Professional conduct
  • Communication and teamwork
Work Environment

This job operates in an indoor medical clinical environment. This role also routinely comes into contact with patients who may have contagious illnesses.

Physical Demands
  • Regularly required to stand, walk, use hands to type, handle, or feel objects, tools, or controls; reach with hands and arms, and talk or hear.
  • Frequently required to sit, stoop, kneel, or crouch and lift and/or move up to 20 pounds.
