Cordial Health is seeking a compassionate and skilled Psychiatric Mental Health Nurse Practitioner (PMHNP) or Physician Assistant with psychiatric experience to provide on-site behavioral health care throughout Brooklyn, New York
.

This part-time position offers a flexible three-day weekday schedule
, meaningful patient relationships, provider autonomy, and extensive clinical and operational support. You will serve adult and geriatric residents who often have limited access to specialized psychiatric services.

Responsibilities include:

  • Conduct comprehensive psychiatric evaluations, consultations, and follow-up visits
  • Diagnose and manage psychiatric and behavioral health conditions
  • Develop individualized, evidence-based treatment plans
  • Prescribe, monitor and adjust psychotropic medications
  • Evaluate treatment response, medication effectiveness, side effects and behavioral stability
  • Collaborate with physicians, nursing staff, therapists, social workers, residents and families
  • Participate in interdisciplinary treatment planning
  • Educate residents, caregivers and staff regarding diagnoses, medications and behavioral interventions
  • Provide compassionate, trauma-informed and resident-centered care
  • Maintain accurate, timely and compliant clinical documentation
Requirements
  • Board-certified PMHNP-BC or Physician Assistant with significant psychiatric experience
  • Active and unrestricted New York professional license
  • Active DEA registration required
  • Strong knowledge of psychiatric assessment, diagnosis, psychopharmacology and treatment planning
  • Ability to manage a psychiatric caseload independently
  • Excellent communication, organization and clinical-reasoning skills
  • Geriatric, skilled nursing, rehabilitation, long-term care or post-acute experience preferred
  • Familiarity with electronic medical record systems preferred
  • Competitive annual compensation of $96,000–$114,000
