Licensed Practical Nurse

Arch Care seeks a compassionate Licensed Practical Nurse to provide hands-on nursing care to seniors and individuals with complex medical and social needs across our continuing care programs. In this mission-driven, faith-based environment, you'll deliver treatments, administer medications, monitor resident conditions, and support interdisciplinary care plans rooted in dignity and holistic service. The LPN collaborates closely with RNs, physicians, therapists, and families, ensuring safe, high-quality care aligned with Catholic values while mentoring aides and contributing to a culture of teamwork and respect.

Responsibilities

• Provide direct nursing care including treatments, wound care, and personal care support
• Administer and document medications and injections in accordance with orders and regulations
• Monitor, record, and report changes in resident/patient condition to RN and provider
• Participate in admission, discharge, and ongoing care planning with the interdisciplinary team
• Maintain accurate, timely nursing documentation in the EHR and paper records as required
• Educate residents, patients, and families on care plans, medications, and health management
• Supervise, mentor, and support CNAs and other support staff in daily care activities
• Ensure compliance with infection control, safety, and regulatory standards
• Support spiritual and psychosocial needs consistent with Arch
