Licensed Practical Nurse

Ontario Center seeks a compassionate Licensed Practical Nurse to provide hands-on care in our skilled nursing and rehab facility. The LPN will deliver medications and treatments, monitor resident status, and support restorative therapies in a resident-centered environment. Responsibilities include assisting with ADLs, documenting care, collaborating with an interdisciplinary team, and promoting safety, dignity, and comfort. This role offers opportunities to grow clinical skills, participate in care planning, and positively impact residents' health and quality of life through attentive, team-based nursing care.

Responsibilities

• Provide direct nursing care to residents including medication administration, treatments, and monitoring of vital signs
• Support physicians and RNs by following care plans and reporting changes in resident condition
• Assist residents with daily living activities while promoting dignity, safety, and comfort
• Document care, assessments, and interventions accurately in medical records and electronic systems
• Collaborate with interdisciplinary team members to coordinate individualized care and rehab goals
• Educate residents and families on care plans, treatments, and health maintenance
• Participate in care conferences, quality improvement, and regulatory compliance activities
• Maintain infection control, safety, and confidentiality standards at all times
• Supervise and support CNAs and other support staff in daily care tasks
• Respond to emergencies and follow facility protocols for acute changes in condition
