LPN, LPN​/LVN, RN Nurse

Rochester Regional Health is seeking a compassionate LPN to deliver high-quality, patient-centered care in our dynamic healthcare network. In this role, you will provide hands-on nursing care, including vital signs, medication administration, wound care, and patient education under RN and provider direction. You will collaborate within a diverse, interdisciplinary team, support admissions and discharges, and maintain accurate documentation. Responsibilities

• Provide direct patient care including vital signs, wound care, and medication administration under RN/physician direction
• Monitor patient status, document care accurately in electronic health records, and report changes promptly
• Assist with patient admissions, discharges, and transfers ensuring smooth care transitions
• Support providers during exams and procedures, preparing rooms, equipment, and supplies
• Educate patients and families on treatments, medications, and self-care practices
• Maintain infection control, safety, and quality standards in all clinical activities
• Collaborate with interdisciplinary team members to develop and implement care plans
• Ensure compassionate, culturally sensitive care aligned with Rochester Regional Health values
