Nursing Supervisor​/Director of Patient Service Assistant

We are located in Queens village and served all five boroughs which provides a broad range of professional and paraprofessional services to clients in their comfort of their homes.

As a Home Care At Its Best Clinical Supervisor, you will be responsible for the direction, coordination, direct supervision and supervisory visits of professional and paraprofessional staff as well as initiating and conducting in-service classes for the caregivers under the direction of the DPS.

• Communicate and confer with patient’s physician in ascertaining the need for home care and in developing, implementing and revising the Plan of Care
• Institute contact with physicians as necessary
• Perform Quarterly visits to professional and paraprofessional staffs as well as their yearly evaluation
• Develop an individualized goal outcome Plan of Care
• Supervise care for all the Agency’s patient census according to the Plan of Care
• Ensure all Physician’s orders are obtained initially by the RN, reviewed as required and communicated appropriately
• Maintain accurate Physical and History assessment of each patient and document any changes in condition by reviewing clinical records such as nurses’ notes and medication records.
• Initiate and conduct in-services for all professional and paraprofessional staffs.
• The right candidate will have a minimum of two years of clinical experience in nursing with a minimum of 1 year of experience in a home health care agency.
• A valid NYS Licensed Practical Nursing License in good standing is required
• This position required your time to be split between the office for reviewing clinical records and out on the field for supervisory periodic visits.
