Licensed Practical Nurse

Holliswood Center in Queens, NY is seeking a compassionate Licensed Practical Nurse to provide resident-centered skilled nursing care. The LPN will administer medications and treatments, monitor vital signs, support rehabilitation goals, and promptly report changes in resident conditions. Working closely with RNs, physicians, and therapists, the LPN helps develop and follow individualized care plans while maintaining accurate documentation. This role supports residents' comfort, dignity, and safety, communicates with families, and collaborates with a multidisciplinary team in a supportive, mission-driven environment focused on quality of life and professional growth.

Responsibilities

• Provide direct bedside nursing care to residents in a skilled nursing and rehab setting
• Administer medications and treatments in accordance with physician orders and facility policies
• Monitor and document vital signs, symptoms, and resident responses to care
• Assist with rehabilitation activities and support residents in achieving care plan goals
• Collaborate with RNs, physicians, therapists, and CNAs to implement individualized care plans
• Accurately complete nursing documentation and maintain medical records in a timely manner
• Report changes in resident condition promptly to supervising nurse or physician
• Educate and support residents and families regarding care, treatments, and routines
• Promote resident dignity, comfort, safety, and infection control standards
• Participate in team meetings, shift reports, and quality improvement initiatives
