• NYS Registration
• CPR Certification

• Malpractice Insurance

  - Previous Employer and 1-Personal)
• Direct Deposit form or Void Check (Must be signed)
• COVID Vaccination (Not required)
• Flue Vaccine (Not required)
• Complete Physical Exam -- (MUST BE WITHIN 6 MONTHS FROM THE DATE OF APPLICATION)

MMR (Measles, Mumps, Rubella or Rubeola) Lab Reports

Chest X-Ray Lab Report (if positive PPD)
