Metropolitan Jewish Health System, Inc. is seeking a Hospice Field Case Manager to visit MJHS Hospice and Palliative Care patients, perform physical and psychosocial assessments, and document visits electronically.
You will develop nursing plans of care and recommend appropriate care based on patient needs, with a schedule that includes one weekend per month and 2-3 on-call shifts per month. A car and escort service are provided for on-call shifts.
