Responsibilities

• Coordinate and monitor patient treatment plans in collaboration with multidisciplinary care teams
• Document assessments, interventions, and outcomes accurately in digital medical records
• Support implementation and use of clinical software tools to improve quality and efficiency of care
• Educate patients and families on treatment plans, medications, and technology-enabled care options
• Follow clinical protocols, safety standards, and regulatory requirements
• Participate in quality improvement initiatives and data-driven care optimization projects
• Collaborate with IT and clinical leadership to provide feedback on system performance and workflow enhancements
