Steuben Center for Rehabilitation and Healthcare in Bath, NY is seeking a compassionate Certified Nursing Assistant to join our Healthcare & Medical Services team. CNAs provide hands-on support with ADLs, mobility, and comfort measures while monitoring residents' condition and reporting changes to nurses. You'll contribute to a resident-focused environment that values dignity, respect, and quality clinical care. Work alongside a collaborative, multidisciplinary team with access to training, mentorship, and growth opportunities within the Centers Health Care network.
NYS CNA certification and a caring, team-oriented attitude are required.
Responsibilities
- Provide direct patient care including bathing, grooming, dressing, toileting, and feeding residents according to individualized care plans
- Assist residents with mobility, including turning, positioning, transferring, and ambulation using proper body mechanics and equipment
- Measure and record vital signs such as temperature, pulse, respiration, blood pressure, weight, and intake/output, and promptly report changes to nursing staff
- Support licensed nurses in carrying out treatments and procedures within scope of practice and under supervision
- Observe and document residents' physical, mental, and emotional status and promptly communicate concerns or changes in condition
- Maintain a clean, safe, and comfortable environment by assisting with housekeeping tasks related to resident care areas and equipment
- Promote resident dignity, independence, and rights through respectful, compassionate, and patient-centered interactions
- Assist with admission, transfer, and discharge of residents, including orientation to the facility and personal care needs
- Follow infection prevention and control protocols and all safety policies to ensure patient and staff safety
- Collaborate with nurses, therapists, and other interdisciplinary team members to support care plans and optimize patient outcomes
- Provide emotional support and companionship to residents and communicate effectively with families and caregivers
- Accurately document care provided in electronic or paper medical records according to facility policy
Required Skills
- Patient care and activities of daily living (ADLs)
- Vital signs measurement and monitoring
- Safe patient handling, transfers, and mobility assistance
- Infection prevention and control practices
- Basic documentation in electronic or paper medical records
- Observation and reporting of changes in patient condition
- Communication with residents, families, and healthcare team
- Time management and task prioritization in a clinical setting
- Basic knowledge of restorative and rehabilitative care techniques
- Understanding of resident rights, dignity, and confidentiality (HIPAA)
