Position: LICENSED PRACTICAL NURSE
Location: Bath

Steuben Center is seeking a Licensed Practical Nurse to join our compassionate Healthcare & Medical Services team. The LPN will provide direct resident care, administer medications, monitor health status, and support care plans under the direction of RNs and physicians. Responsibilities include documenting care, collaborating with the multidisciplinary team, and communicating with residents and families. Ideal candidates are resident-focused, empathetic, and detail-oriented, with strong clinical and teamwork skills.

Responsibilities

  • Provide direct nursing care to residents in a skilled nursing and rehabilitation setting under the supervision of an RN.
  • Administer medications and treatments safely and accurately in accordance with physician orders and facility policies.
  • Monitor and document vital signs, pain levels, and responses to treatments, promptly reporting significant changes in condition.
  • Perform wound care, dressing changes, and other clinical procedures within LPN scope of practice.
  • Support residents with activities of daily living and promote comfort, dignity, and independence.
  • Maintain accurate, timely documentation in the electronic medical record, including assessments, interventions, and resident outcomes.
  • Follow all infection control, safety, and fall prevention protocols to protect residents, staff, and visitors.
  • Participate in developing and updating individualized care plans in collaboration with the interdisciplinary team.
  • Educate residents and families on medications, treatments, and health management in an understandable and compassionate manner.
  • Coordinate and communicate with CNAs, RNs, therapists, and physicians to ensure continuity and quality of care across shifts.
  • Participate in ongoing training, in-service education, and quality improvement initiatives as required by the facility.
