Position Overview

We are seeking a dedicated Licensed Practical Nurse (LPN) to support an outpatient primary care team in Ellenville. The LPN will work closely with providers and other clinical staff to provide patient care, assist with clinical procedures, obtain vital signs, administer medications as directed, and support efficient day-to-day operations of the health center.

Responsibilities
  • Obtain and document patient vital signs and medical histories.
  • Prepare patients for examinations and procedures.
  • Assist physicians, NPs, and PAs with patient care and clinical procedures.
  • Administer medications, injections, and treatments as ordered and within scope of practice.
  • Perform routine nursing tasks and follow established clinical protocols.
  • Provide patients with basic health education and instructions.
  • Assist with preventive care and chronic disease management activities.
  • Document patient information accurately in the electronic medical record.
  • Assist with follow-up calls, referrals, and coordination of patient care.
  • Communicate patient needs and clinical information to the appropriate members of the care team.
  • Maintain a clean, safe, and organized clinical environment.
  • Provide respectful, culturally sensitive care to a diverse patient population.
Qualifications

Active New York State LPN license. Graduate of an accredited practical nursing program. Current BLS certification. Prior outpatient, primary care, community health, or FQHC experience preferred. Strong communication and interpersonal skills. Ability to work effectively as part of a multidisciplinary healthcare team. Bilingual skills are a plus.
