Nurse Case Manager
Serve as Nurse Case Manager responsible for analyzing problems associated with providing health care to patients and/or beneficiaries for the Military Treatment Facility (MTF).
Conduct comprehensive assessments of beneficiary's health needs to develop a plan of care.
Educate both patient and members of the healthcare delivery team about case management, healthcare and treatment options, community resources, insurance benefits, psychosocial concerns, etc. so that informed decisions can be made.
Consult and coordinate with other multi-disciplinary care professionals, agencies, organizations and other ancillary support systems.
Serve as a liaison between the patent and the health care professionals.
Update follow up assessment information with the patient's physical and emotional condition within their electronic record.
Address current issues and improve overall processes by contributing to daily multi-disciplinary team meetings.
