Assistant Director of Nursing (ADON) — Skilled Nursing Facility

Location: Flushing, Queens, NY
Employment Type: Full-Time | On-Site
Salary: $110,000 – $120,000 annually

About the Role

We are seeking an experienced Assistant Director of Nursing (ADON) to join the leadership team at a skilled nursing and rehabilitation facility in Flushing, Queens. Reporting directly to the Director of Nursing, the ADON supports the daily clinical and administrative operations of the nursing department, ensures compliance with NYSDOH and CMS regulations, and helps lead a team committed to high-quality resident care.

Key Responsibilities
  • Assist the DON in the overall management, supervision, and coordination of nursing services across all shifts
  • Serve as acting DON in the Director's absence
  • Monitor resident care delivery to ensure compliance with federal, state, and local regulations, including 10 NYCRR and CMS Conditions of Participation
  • Support survey readiness and participate in state surveys, plans of correction, and follow-up audits
  • Oversee infection prevention and control activities in partnership with the IP nurse
  • Participate in QAPI initiatives, clinical audits, and performance improvement projects
  • Review and monitor clinical documentation, care plans, incident reports, and MDS accuracy
  • Supervise, mentor, and evaluate RNs, LPNs, and CNAs; support onboarding, orientation, and ongoing in-service education
  • Assist with nursing schedules, staffing ratios, and payroll review to maintain appropriate coverage
  • Investigate and respond to resident, family, and staff concerns; participate in grievance resolution
  • Collaborate with physicians, therapy, social services, dietary, and administration on interdisciplinary care planning
  • Assist with clinical review of admissions and readmissions
  • Participate in on-call rotation as needed
Qualifications
  • Active and unrestricted New York State RN license (required)
  • Bachelor's degree in Nursing (BSN) preferred;
    Associate's degree with equivalent experience considered
  • Minimum 2 years of nursing management or supervisory experience in a skilled nursing or long-term care setting (required)
  • Working knowledge of NYSDOH regulations, CMS guidelines, MDS 3.0, and survey processes
  • Strong clinical assessment, documentation, and leadership skills
  • Proficiency with electronic health records (Point Click Care  preferred)
  • Current BLS certification
  • Excellent communication and interpersonal skills; bilingual (Mandarin, Cantonese, Korean, or Spanish) a plus given the community served
