Pay: $30.00 - $35.00 per hour

People Care has served New York patients and families for 50 years. We employ roughly 4,300 caregivers across New York City and Long Island. This seat sits directly between the patient and the nurse who visits them — you decide what gets scheduled, what gets escalated, and what does not fall through.
WHAT YOU OWN
- Case coordination
. Your assigned patient cases end to end — care plans, service needs, and changes in condition. You are the liaison among patients, families, physicians, field staff and agency teams. - Nurse visit scheduling.Visits scheduled against patient need, authorization and staff availability. Nurses assigned by qualification, geography and continuity of care — the same nurse where it matters clinically.
- Service hour review
. Reviewing cases to determine eligibility and the need for service hour reviews, and reading clinical documentation to support care planning an authorized services. - Coverage integrity
. Missed visits, call-outs and coverage gaps caught early and closed — not discovered after the fact. - Escalation
. Changes in patient condition, staffing gaps and service issues raised to clinical leadership while there is still time to act on them. - Documentation.Accurate, timely records in the EMR and agency systems, to LHCS standard.
- Patient and caregiver support
. Concerns resolved directly, with follow-through the family can feel.
WHO YOU ARE
You are someone who cares deeply about patients and the caregivers who serve them. You are transparent — you surface a coverage problem early rather than hoping it resolves itself. You go beyond what the task asks when a patient's care depends on it. You row together with coordination, intake and clinical rather than working around them. And you bring the credentials to back it up:
- Current NYS LPN license
in good standing. This is non-negotiable. - Home care judgment
. Experience in home care, case management, or scheduling LHCSA setting preferred. - Care coordination fluency.Familiarity with LHCSA operations, care coordination workflows and service authorization.
- Organization under pressure.Multiple priorities, a moving schedule, and a phone that keeps ringing — handled without losing the thread.
- Communication.Professional and clear with patients, families, nurses and physicians alike.
- Systems comfort.EMR and scheduling platforms. Strong general computer skills.
- Presence. On-site in our Brooklyn office.
NICE TO HAVE
- Experience coordinating home health or personal care services
- Knowledge of Medicaid, managed care, and home care documentation standards
