Registered Nurse Assessment Coordinator

The Allure Group is looking for an RN Assessment Coordinator to join the team at our Skilled Nursing Facility in Brooklyn, NY!

As an RN Assessment Coordinator at The Allure Group, you would be part of a winning team doing meaningful work to improve the quality of life, health, and comfort of residents.

Assessment Coordinator Responsibilities
  • Conduct and coordinate the completion of MDS assessments
  • Maintain and update all care plans and assessments
  • Examine and review residents’ medical history
  • Inform other nursing staff personnel of any persistent issues
  • Participate in the admission process while assessing prospective residents’ nursing needs and appropriate placement
  • Coordinate case management process
  • Ensure that the facility adheres to all regulatory requirements and standards of practice in relation to the MDS care planning process
  • Cooperate with inter-departmental personnel to ensure services can be adequately maintained to meet the needs of the residents
RN Assessment Coordinator Requirements
  • Valid, current RN license
  • Previous experience in long-term care or skilled nursing facility
  • One-year job-related experience in the MDS process
  • Familiarity with various computer software
  • COVID-19 Vaccination
