Administer medications and treatments as prescribed by physicians, including oral, intravenous, subcutaneous, and intramuscular routes.
Monitor and record vital signs, observe residents for changes in condition, and report significant findings to the supervising RN or physician promptly.
Provide wound care, including dressing changes, and manage other treatments such as IV therapy, catheter care, tracheostomy care, and ostomy care.
Assist residents with activities of daily living (ADLs) such as bathing, dressing, grooming, mobility, and feeding.
Collect specimens for laboratory testing and perform basic diagnostic procedures.
Maintain accurate, timely documentation in residents’ medical records in compliance with facility policies and regulatory requirements.
Supervise and delegate tasks to Certified Nursing Assistants (CNAs) and other support staff as appropriate.
Provide emotional support to residents and their families, and communicate effectively about care plans and health status.
Ensure a safe, clean, and comfortable environment while adhering to infection control protocols.

Current Basic Life Support (BLS) / CPR certification.
Minimum 1 year of nursing experience preferred, ideally in skilled nursing, long-term care, subacute rehab, or geriatric care.
Strong clinical skills in medication administration, wound care, and patient assessment.
Excellent communication, interpersonal, and organizational skills.
Ability to work effectively as part of a team and supervise unlicensed personnel.
Must pass a background check, drug screening, and meet all NY state and facility health requirements (e.g., vaccinations, TB test).

Facility assignment location:
Manhattan, Brooklyn, Queens, Bronx.

Summary

Record and maintain accurate nursing documentation and electronic health records (EHR).
Provide wound care, catheter care, tube feeding assistance, and other nursing procedures within the LPN scope of practice.
Supervise and support Certified Nursing Assistants (CNAs) in providing daily resident care.
Respond to resident emergencies and participate in infection control and safety procedures.
Communicate effectively with residents, families, physicians, and interdisciplinary team members.
Ensure compliance with New York State Department of Health regulations and facility policies.
