Licensed Practical Nurse (LPN)
The primary functions of a Licensed Practical Nurse (LPN) include assisting with the assessment, planning, implementation, and evaluation of the care of all patients, regardless of age, during the shift. The LPN is responsible for meeting the established standards of skilled nursing practice while collaborating with the care team, participating in interdisciplinary care, and advocating for patients.
Responsibilities- Effectively provide person-centered care and services.
- Administer medications and treatments under the supervision of a Registered Professional Nurse in accordance with Facility Policy.
- Take and report patients' vital signs including respiration, temperature, blood pressure, and heart rate, while also notifying the team of changes in patient conditions.
- Practice ethically and professionally within the scope; advocate for patient well-being.
- Maintain clinical competence specific to the care area, including completion of required education within stated timelines.
- Coordinate care with the interdisciplinary team and document appropriately in EMR/Point Click Care.
- Keep nursing personnel and the responsible RN informed regarding changes in conditions of patients or other pertinent observations.
This position is full-time with a schedule of 3:00 PM to 11:30 PM, working five evenings per week. Every other weekend and holiday are mandatory, along with potential floating to other units as staffing permits. The position may also require transitioning to a CNA role as staffing needs dictate.
Required Experience / Certifications / Licensure- Valid LPN license.
- Experience in Point Click Care is preferred.
- Completion of a TB test, Hepatitis B vaccination, MMR, Varicella, Tdap, and Influenza vaccine as part of the onboarding process.
- Submit a driver's license or state complete an agency background check and drug screen.
