Description:

The role requires the ability to review health status questions. Evaluations and determinations must be made based on UAS criteria for enrollment. Candidates should be proficient with technology in the workplace. Knowledge of nursing theory, principles, and practices is essential. Understanding state laws and regulations governing professional nursing practices is required. Excellent organizational and interpersonal skills are necessary. Strong written and verbal communication skills are essential.

Candidates should be able to effectively communicate with multi-generational consumers. Sensitivity to the concerns of others is important. Ability to perform comfortably in a fast-paced, deadline-oriented work environment is needed. Candidates must be capable of executing many complex tasks simultaneously. The ability to work independently is crucial.

Job Responsibilities:

Perform initial in-home pre-evaluations of patients, including evaluating health status, strengths, care needs, and preferences. Guide the development of individualized long-term care service plans. Review consumer medical documentation and health referral forms related to the case. Enter evaluation data into electronic forms and transmit as required. Document any concerns, conflicting information, or issues that arise during evaluations. Emphasize continuity of care to reduce fragmentation and gaps in treatment plans.

Discuss health care options, necessary supports, and service vendor options with consumers. Initiate communication for enrollment into managed care health plans. Enhance communication and collaborative relationships with interdisciplinary care team members. Collect quality review data and required documentation for outcome measurements. Identify opportunities for health promotion and illness prevention. Maintain a comprehensive knowledge of community resources and network services for the target population.

Meet all standards for this position as outlined in the corresponding performance criteria. Perform other duties as assigned by the Regional Nurse Manager or project management.
