Licensed Practical Nurse

Arch Care seeks a compassionate Licensed Practical Nurse to provide direct clinical care to seniors and individuals with complex medical and social needs in our faith-based Continuing Care Community. The LPN will administer medications and treatments, monitor and document patient status, support care plans, and collaborate with RNs, physicians, therapists, and caregivers across settings. This role promotes dignity, safety, and comfort while supporting families, mentoring aides, and upholding Catholic, mission-driven values of respect, service, and holistic, person-centered care.

Responsibilities

• Provide direct nursing care to residents/patients under RN and physician direction
• Administer medications and treatments safely and accurately
• Monitor, document, and report changes in patient condition
• Implement and support individualized care plans
• Collaborate with RNs, physicians, therapists, and interdisciplinary team members
• Supervise and support nursing assistants and other support staff
• Educate patients and families on care, treatments, and health maintenance
• Maintain accurate, timely clinical documentation and records
• Adhere to infection control, safety, and quality standards
