Join an established New York healthcare organization providing skilled home health services across Brooklyn, Manhattan, and Queens. The organization supports patients across the lifespan, including well-baby, school-aged, postpartum, adult, and geriatric populations.

We are hiring full-time Home Health RNs for weekday field-based positions. Nurses will complete clustered home visits, with opportunities available in Brooklyn, Brooklyn/Manhattan, and Queens.

What You’ll Be Doing

• Complete approximately 5–6 skilled home health visits per day within an assigned geographic territory
• Perform admissions, initial assessments, OASIS documentation, and ongoing skilled nursing visits
• Provide care to a mix of pediatric, postpartum, adult, and geriatric patients
• Develop, implement, and document individualized plans of care in coordination with physicians and other clinical disciplines
• Educate patients and families on disease management, self-care, medications, and prevention strategies

What You Need

• Active New York State Registered Nurse license
• Reliable transportation and ability to travel throughout the assigned territory
