CASE MANAGER

VNS Health is seeking a Clinical Case Manager to coordinate compassionate, patient-centered care for individuals receiving home and community-based services. In this role, you will conduct comprehensive clinical assessments, develop and manage individualized care plans, and collaborate with an interdisciplinary team to optimize patient outcomes. You will support patients and families through education, resource coordination, and ongoing advocacy. Responsibilities

• Conduct comprehensive clinical assessments for home and community-based patients
• Develop, implement, and update individualized care plans
• Coordinate services across providers, caregivers, and community resources
• Monitor patient progress and adjust interventions to optimize outcomes
• Ensure compliance with regulatory and payer requirements
• Document all care activities accurately in the EHREducate patients and families on conditions, treatments, and resources
• Collaborate closely with the interdisciplinary team to support safe transitions of care
• Participate in quality improvement and population health initiatives
