Clinical Team Manager

VNS Health in New York City seeks a Clinical Team Manager to lead a home and community-based care team. Responsibilities

• Lead and supervise a multidisciplinary clinical team delivering home and community-based care
• Oversee patient caseloads, assignments, and daily workflow to ensure timely, high-quality services
• Monitor clinical outcomes, quality metrics, and documentation for regulatory compliance
• Collaborate with physicians, social workers, and caregivers to develop and adjust care plans
• Provide coaching, performance feedback, and professional development for clinical staff
• Support hiring, onboarding, and training of new team members
• Ensure accurate and timely use of EHR and reporting systems
• Identify and address clinical risks, escalations, and patient safety concerns
• Drive quality improvement initiatives and best practices in patient-centered care
• Foster a culture of compassion, integrity, collaboration, and continuous improvement
