This is a remote/hybrid position with opportunities for clinical work, including assessment, medication administration, and injections, based on program needs.

Responsibilities
  • Conduct comprehensive health assessments and physical health reviews for patients enrolled in outpatient and residential substance use and mental health treatment services (via telehealth and in-person as needed)
  • Collaborate with an interdisciplinary team including medical, psychiatric, counseling staff and peers to support integrated care planning
  • Assist addiction medicine and psychiatric providers with medication management, including monitoring adherence, effectiveness, and side effects
  • Support the administration and coordination of specialty medications (e.g., MAT/MOUD such as buprenorphine, naltrexone, and long-acting injectable medications)
  • Provide nursing services, including injections and medication administration, as required
  • Complete administrative applications ie SCAT application and prior authorizations and liaise with pharmacies and insurance providers to ensure timely medication access
  • Perform medication reconciliation and audits, including coordination with residential treatment programs to ensure continuity and accuracy of medication regimens
  • Conduct triage of patient needs, including responding to clinical concerns and determining appropriate level of care or follow-up
  • Provide outreach and follow-up communication (emails, phone calls, telehealth visits, care coordination) to support engagement, adherence, and retention in treatment
  • Educate patients on medications, health conditions, and wellness strategies to support recovery.
  • Educate staff on topics related to mental and physical health and medications
  • Maintain accurate and timely clinical documentation in accordance with regulatory and organizational standards
  • Ensure compliance with OASAS/OMH regulations, telehealth requirements, and all applicable federal, state, and organizational policies
  • Participate in quality improvement initiatives and interdisciplinary team meetings (virtual and in-person as needed)
Qualifications
  • Current, unrestricted Registered Nurse (RN) license in New York State
  • Minimum of 3 years of nursing experience; experience in addiction medicine, behavioral health, outpatient, or residential care preferred
  • Knowledge of medication for addiction treatment (MAT/MOUD) and substance use disorders strongly preferred
  • Familiarity with OASAS and OMH regulations and outpatient/residential treatment models preferred
  • Experience or comfort with telehealth/remote care delivery and related technologies
  • Ability and willingness to perform in-person clinical duties, including injections and medication administration as needed
