Salary: $140,000–$160,000 annually
Schedule: Full-Time | Monday–Friday | Availability between 8:00 AM–6:00 PM
A medical group providing primary care services to nursing homes is seeking an experienced Nurse Practitioner (NP) or Physician Assistant (PA) to provide ongoing primary care coverage at a skilled nursing facility in the Canarsie section of Brooklyn.
This is a full-time opportunity for a provider who is comfortable managing a consistent patient panel in a nursing home environment. The provider will work closely with nursing staff and facility leadership to manage both routine and acute medical needs while ensuring continuity of care.
Position OverviewThe NP or PA will be responsible for managing a panel of geriatric nursing home residents, conducting regular and urgent visits, managing chronic conditions, and coordinating care with the facility's clinical team.
Clinical Responsibilities- Provide primary care services to geriatric nursing home residents
- Conduct routine, follow-up, and urgent patient visits
- Evaluate and manage acute medical concerns
- Manage patients with multiple chronic medical conditions
- Review medications, labs, and treatment plans
- Coordinate care with nursing staff, specialists, and facility leadership
- Participate in CCM and RPM programs when applicable
- Maintain accurate and timely clinical documentation
- Document patient encounters using Sigma Care or PCC
- Active New York Nurse Practitioner or Physician Assistant license
- Active Medicare and Medicaid provider
- Experience working with geriatric patients preferred
- Skilled Nursing Facility (SNF) or long-term care experience strongly preferred
- Previous RN experience in a nursing home or long-term care setting is highly valued
- Strong clinical judgment and ability to work independently
- Comfortable managing a consistent nursing home patient panel
- $140,000–$160,000 annual salary
- Full-time, Monday–Friday
- Availability between 8:00 AM–6:00 PM
