Nurse Practitioner – Nursing Home Primary Care

Location: Brooklyn, NY
Schedule: Full-Time
Salary: $130,000–$150,000 annually, plus RVU incentives

Med Staff Group is seeking a full-time Nurse Practitioner to provide primary care services to residents of a nursing home in Brooklyn.

This opportunity is ideal for an NP with nursing-home or geriatric experience. Candidates who are newer Nurse Practitioners but have previous nursing-home experience as a Registered Nurse are encouraged to apply.

Responsibilities:

• Provide primary care to nursing-home residents
• Conduct routine, follow-up, and urgent patient visits
• Diagnose and manage acute and chronic medical conditions
• Review medications, laboratory results, and treatment plans
• Coordinate care with physicians, nursing staff, specialists, and facility leadership
• Complete accurate and timely clinical documentation
• Participate in care planning and help ensure continuity of care

Qualifications:

• Active New York State Nurse Practitioner license
• Active New York State Registered Nurse license
• Active Medicare and Medicaid provider  required
• Nursing-home, skilled nursing, long-term care, or geriatric experience preferred
• Previous nursing-home experience as an RN will be considered
• Strong clinical judgment, communication, and documentation skills
• Comfortable working independently and collaborating with an interdisciplinary team

Compensation and Benefits:

• Base salary of $130,000–$150,000
