Role Description The Lead Psychiatric Mental Health Nurse Practitioner will provide comprehensive psychiatric evaluation, diagnosis, and treatment to residents in nursing homes in the NY Metro area. This full-time, on-site role includes conducting psychiatric assessments, prescribing and managing psychotropic medications, and delivering ongoing follow-up care. The PMHNP will collaborate closely with attending physicians, nursing staff, social workers, and families to develop and adjust individualized treatment plans.

Daily responsibilities also include documentation in the medical record, participation in care plan meetings, and adherence to evidence-based guidelines and regulatory requirements. The lead practitioner will help guide clinical standards, support quality improvement initiatives, and contribute to a culture of compassionate, patient-centered care.

Qualifications

  • Current PMHNP (Psychiatric Mental Health Nurse Practitioner) certification and active, unrestricted nurse practitioner license in the state of practice.
  • Experience in geriatric psychiatry, long-term care, or working with nursing home residents.
  • Strong skills in psychiatric assessment, diagnosis, and psychopharmacology management.
  • Excellent communication, documentation, and interpersonal skills for working with patients, families, and facility staff.
  • Commitment to compassionate, trauma-informed, and culturally sensitive care.
  • Ability to work independently, manage a clinical caseload, and prioritize tasks in a fast‑paced environment.
  • Familiarity with electronic health records and regulatory standards in long‑term care and behavioral health.
