Registered Nurse

Location: Manhattan (home base) & community-based

Are you an RN passionate about supporting individuals with IDD and behavioral health needs
? This is a great opportunity to provide meaningful, community-based care while working with a high level of clinical autonomy and independence.

You’ll provide a combination of clinical nursing, care coordination, hospital discharge support, home visits, assessments, and community-based services
, while collaborating with an interdisciplinary team.

• Conduct nursing assessments and screen individuals for program admission
• Coordinate hospital discharges and support transitions back to the community
• Conduct home/residential visits and provide ongoing clinical support
• Oversee medications, health monitoring, chronic disease management, wound care, and other nursing needs
• Coordinate care with hospitals, providers, families, and interdisciplinary teams
• Support individuals with intellectual/developmental disabilities (IDD) and severe mental illness (SMI)
• Travel independently throughout the five boroughs, primarily via public transportation
• Maintain accurate documentation and compliance with OPWDD/OMH/DOH requirements

• 2+ years of nursing experience
• IDD, behavioral health, community health, or crisis experience preferred
• Strong clinical assessment and care coordination skills
• Excellent clinical judgment and ability to work independently
• Strong communication, organization, and documentation skills
