At Home Care Partners, a Licensed Home Care Agency, is seeking a compassionate and dependable Home Care Field Registered Nurse (RN) to provide high-quality nursing care to clients throughout Chenango County, New York
.

This position is ideal for an RN who enjoys working independently, providing personalized patient care, and making a meaningful impact outside of a traditional hospital or facility setting.

As a Home Care Field RN
, you will travel to client homes and community-based settings throughout the assigned service area to complete nursing assessments, oversee patient care, conduct aide supervisions, and develop individualized Plans of Care.

• Travel to client homes and community-based settings throughout Chenango County

• Conduct comprehensive in-home and community-based nursing assessments

• Perform head-to-toe evaluations, including medication and diagnosis reviews

• Complete aide supervisions and provide clinical guidance to caregivers

• Develop, review, and update individualized Plans of Care (POC)

• Orient aides to each client's Plan of Care and specific care needs

• Monitor client conditions and report changes or concerns as appropriate

• Communicate effectively with patients, families, caregivers, aides, and office staff

• Complete accurate and timely clinical documentation

• Help ensure services are delivered safely and according to each client's individual needs

• Maintain compliance with applicable healthcare regulations and company policies

• New York State Registered Nurse (RN) license

• Ability and willingness to travel throughout Chenango County and assigned service areas

• Medical Professional Liability insurance

• Strong nursing assessment and clinical documentation skills

• Comfortable using Gmail and Microsoft Word/Excel

• Excellent communication and interpersonal skills

• Ability to work independently in a field-based environment

• Professional, reliable, organized, and compassionate

• Must meet all company hiring and onboarding requirements

• Previous home care, home health, outpatient nursing, or community health experience

• Familiarity with Medicaid, Medicare, or MLTC plans

• Experience conducting patient assessments and aide supervisions

• Experience developing, reviewing, or updating Plans of Care

• Strong clinical assessment and care coordination skills

• Bilingual in Spanish and English

• Comfortable working with patients from diverse backgrounds and with varying healthcare needs

Salary: $85,000 per year
