About This Role

The Care Manager - RN in the Medicine Units at New York-Presbyterian is responsible for managing complex cases and ensuring continuity of care for patients. This role is ideal for a driven and tenacious individual who can lead nursing practice in a respected university medical center.

• Pay: $134,900 - $168,000/year
• Location: New York, NY
• Facility: New York-Presbyterian
• Schedule: Full-Time, Day shift

• Manage complex cases and serve as a liaison between acute care and critical care teams.
• Coordinate efforts of social workers, feeding specialists, PTs, OTs, cardiologists, intensivists, and neonatologists.
• Direct all aspects of inpatient care, ensuring optimal length of stay.
• Anticipate outpatient service needs, setting appointments and consultations prior to discharge.

• Bachelor's degree in Nursing (BSN) required

• 3 to 5 years of healthcare experience in an acute care setting
• 2 years of case management/care coordination experience in an inpatient, HMO, insurance, or other outpatient setting preferred
• 1 year of leadership experience in a healthcare setting preferred

• Current NYS Registered Nurse (RN) licensure required
