CNA – LTC​/SNF

Job Summary:

Provide direct patient care in a long-term care or skilled nursing facility setting under the supervision of nursing staff. Ensure patient comfort, safety, and well-being through routine and specialized assistance.

Responsibilities:

• Assist patients with activities of daily living such as ambulation, feeding, turning, and repositioning.
• Perform basic nursing tasks including vital signs measurement, urine/stool collection, and Foley catheter care.
• Maintain a clean and safe environment by changing linens, bed shampooing, and equipment cleaning.
• Follow infection control protocols, including donning and removing PPE, and adhering to isolation precautions.
• Observe and report changes in patient conditions, and document care provided accurately.
• Support the use of mobility aids, specialty beds, and assistive devices to enhance patient comfort and safety.

Qualifications:

• Hold a valid NY State CNA license in good standing.
• Demonstrate at least 1 year of hands-on experience in LTC/SNF settings.
• Be willing to work evening shifts (14:00-22:00) and float as needed.
• Have experience with EPIC charting system and module billing.
• Capable of lifting and repositioning patients using proper body mechanics.
• Adhere to background and compliance policies as required.
