Care Nurse; RN Home Care, Bronx

Overview

The SOC (Start of Care) Nurse is responsible for providing comprehensive nursing assessments and initiating home care services for patients transitioning into care. The SOC Nurse conducts Start of Care visits, develops and implements individualized plans of care, coordinates multidisciplinary services, and ensures compliance with regulatory, agency, and payer requirements. The role requires strong clinical judgment, care coordination, documentation, and communication skills to support safe and effective patient outcomes.

Works under moderate supervision.

• Practice independently in the community as part of an interdisciplinary care team.
• Deliver personalized nursing and care management to patients in their home or care facilities.
• Constantly evaluate evolving patient needs and respond with plan of care adjustments.

BLS certification preferred Current New York State Registered Nurse license and registration required

Associate's Degree in Nursing required Bachelor's Degree in Nursing from an accredited nursing program preferred

Minimum one year of recent clinical nursing experience in home care, medical-surgical, acute care, or related setting. required

Experience completing OASIS assessments and Start of Care visits preferred. preferred

Demonstrated knowledge of home care regulations and reimbursement practices preferred. preferred

Strong computer and electronic medical record (EMR) skills required. required

Valid driver's license or government-issued photo  based on operational needs. preferred
