Case Manager Registered Nurse - Field in Suffolk, Brooklyn, Kings & Queens County NY

About This Role

The Registered Nurse Case Manager is a field-based role responsible for conducting member assessments, annual reassessments, and change-in-condition visits for new NY Better Health enrollees. This position is ideal for experienced nurses who excel in clinical assessment and care coordination.

• Pay: $66,575 - $142,576/year
• Location: New York, NY
• Schedule: Full-time, Monday-Friday, 8:30 a.m.
  
  -5:00 p.m. EST
• Travel: Up to 75% travel required

• Conduct member assessments for new NY Better Health applicants.
• Perform annual, semiannual, and change-in-condition assessments.
• Submit complete, accurate documentation within 24 hours of each visit.
• Communicate findings to interdisciplinary teams to support care planning.
• Maintain punctuality and reliability for 2-3 daily home visits.
• Operate effectively in a remote/telephonic environment while collaborating with cross-functional partners.

• Associate's degree in Nursing required, Bachelor's preferred

• Case Management Experience
• Strong clinical assessment, critical thinking, and care coordination skills

• Current, unrestricted RN license in NY required
• Must possess reliable transportation
