• Assess the patient's physiologic health status through interviews and chart reviews.
• Determine mobility, sensory deficits, and skin condition to adjust care plans.
• Communicate and document patient's health status and plan of care.
• Develop criteria for measuring patient outcomes and identify potential problems.
• Formulate and document the plan of care, coordinating with healthcare team members.
• Implement the plan of care while ensuring patient safety and comfort.
• Administer prescribed treatments and medications accurately and safely.
• Collaborate with care team members to plan and carry out treatment regimens.
• Participate in patient and family education regarding health conditions and care.
• Perform related duties as required.

Education:

• Graduate from an accredited School of Nursing;
  Bachelor's Degree in Nursing preferred.

Experience:

• 2 years of Med/Surg or Telemetry Experience highly preferred.

Licenses &

Certifications:

• Current license to practice as a Registered Professional Nurse in New York State.
• BCLS certification required; ACLS preferred.
