Hospice Care Registered Nurse, Field Queens

About This Role

The Hospice Care Registered Nurse provides comprehensive nursing care for patients in the field, utilizing the nursing process to assess, plan, implement, and evaluate care. This role is ideal for experienced nurses who are passionate about hospice and home care.

  Full-Time, Day shift

• Conduct pre-admission screening and initial nursing health assessments on new patients.
• Develop nursing plans of care and goals based on patient needs.
• Provide nursing care including supervision of diet, medication, and activities of daily living.
• Evaluate the provision of nursing care, including therapy and nutrition counseling.
• Train and evaluate home health aides and personal care workers in the patient's home.
• Recommend the rental or purchase of medical equipment and supplies for patients.
• Provide ongoing evaluation of equipment for defects and repairs.
• Participate in on-call coverage system on a rotating basis.
• Participate in the ongoing Quality Improvement Program.
• Perform related duties as required.

• Education: Graduate from an accredited School of Nursing;
  Bachelor’s Degree in Nursing preferred.
• Experience: 2+ years of nursing experience in med/surge, hospice, or home care preferred. Minimum one year of experience in acute care – med/surg strongly preferred.

  Current License to practice as a Registered Professional Nurse in New York State required. Specialized certifications as needed.
