• Provide direct nursing care to residents in the long-term care setting in alignment with the plan of care.
• Administer medications and treatments under supervision, monitoring for adverse effects.
• Monitor resident conditions, perform routine assessments, and communicate changes to the supervising RN.
• Assist with activities of daily living, hygiene, mobility, nutrition, and wound care as indicated.
• Document care and progress accurately in the resident charting system and update care plans as needed.
• Collaborate with the interdisciplinary team to ensure safe, comprehensive, high-quality care for residents.

• Active Licensed Practical Nurse license in New York.
• Graduation from an approved practical nursing program.
• Current Basic Life Support (BLS) or CPR certification.
• Prior experience in long-term care or geriatric settings preferred.
• Ability to work in a fast-paced long-term care environment and follow established care plans and policies.
• Must be able to pass background check and drug screening.
