Licensed Practical Nurse; LPN; Long Term Care

LVN – LTC

Schedule

5 days per week, 8-hour shifts with a standard shift time of 06:45 - 15:15. Weekend shifts required every other weekend upon facility request.

Assignment Details

Contract length is determined by facility needs. Compliance process may take 1.5 to 2 weeks.

Requirements

• Active NY State License (no license hits allowed)
• Minimum of 1 year of Long-Term Care (LTC) experience required
• BLS certification required
• Must be able to pass a medication administration test and clinical observation
• Strong attendance and punctuality required

Responsibilities

• Administer medications and treatments as prescribed.
• Perform medication passes accurately and safely.
• Monitor and report changes in resident condition.
• Complete timely and accurate documentation.
• Provide nursing care in accordance with facility policies.
• Supervise CNAs on assigned units.

Compliance / Notes

Must provide proof of license at time of submission. RTO only approved at time of submission; no holidays can be taken off. Call-outs must be given with ample notice, more than 24 hours prior. Parking is free.
