LPN – LTC

Schedule:

Mid shift, 5x8-hour (15:00 - 00:00)

Assignment Details:
Provide care and service to a minimum of 20 and a maximum of 40 residents.

• Current LPN License
• Experience in long-term care preferred
• Basic life support (BLS) certification

• Provide direct patient care, including assistance with activities of daily living.
• Monitor residents' well-being and report changes to the supervising RN or physician.
• Administer medications as prescribed and document accurately.
• Provide wound care and assist in specimen collection for lab testing.
• Supervise and direct CNAs in daily resident care tasks.
• Participate in care planning and interdisciplinary team meetings.
