The Field Nurse in the Home Care Division at NYU Langone Health provides safe, quality, therapeutic age-appropriate nursing care using evidence-based practice. This role is ideal for a nurse who excels in clinical assessment, decision-making, and patient education while coordinating care with an interdisciplinary team.

• Pay: $116,515 - $141,265/year

• Schedule: Full-Time, Day shift

• Completes a comprehensive nursing assessment including physical, functional, psychosocial, emotional, cognitive, cultural, and environmental factors.
• Derives diagnoses and needs from assessment data, collaborates with patients and healthcare teams to develop individualized care plans.
• Coordinates care by interacting with other healthcare team members to optimize patient outcomes and satisfaction.
• Delivers direct nursing care, ongoing assessment, teaching, and training to patients and caregivers.
• Identifies deficits and barriers to effective patient education and employs strategies to enhance engagement.
• Maintains accurate and timely documentation reflecting assessments, diagnoses, plans of care, and evaluations.
• Assists agency leadership with achieving corporate goals and participates in quality improvement initiatives.
• Performs other duties as assigned.

Education:

• Bachelor's degree in Nursing required

Experience:

• 1-2 years in an acute care facility or home health, hospice, and long-term home health care program

Licenses &

• Current Registered Professional Nurse License in New York
