LPN – LTC

Schedule:

Shift hours: 5x8, 3:00 PM - 11:30 PM. Weekend requirement:
Every other weekend. Must be available to work every other holiday, if needed.

Assignment Details:
Contract length: 13 weeks.

Requirements:

• Active New York Single State LPN license (No pending licenses accepted).
• BLS certification required.
• EPIC experience required.

Responsibilities:

• Provide direct patient care and assist with daily living activities.
• Administer medications and treatments as prescribed.
• Monitor patient health and report changes to the nursing staff.
• Document patient care and ensure accurate charting.
• Collaborate with healthcare team members to develop and implement care plans.

Compliance / Notes:
The client may cancel (call off) up to three shifts during a 13-week contract period without incurring charges for the canceled hours. RTO, block, and special scheduling requests must be submitted upon submission.
