LVN​/LPN – LTC​/SNF

Job Summary:

Seeking an experienced LPN/LVN to provide skilled nursing care within a nursing facility. The role involves charting, patient care, and floating among units, ensuring quality support for residents.

Responsibilities:

• Administer and document patient care using EPIC electronic health record system.
• Float within skillset to various units as needed, maintaining flexibility and adaptability.
• schedule from 3:00 PM to 11:30 PM, including every other weekend and holiday coverage.
• Submit RTO, block, and special scheduling requests upon submission.
• Adhere to call-off policies, with up to three shift cancellations permitted within 13 weeks.
• Maintain compliance with unit ratios and ensure accurate documentation and communication.

Qualifications:

• Hold an active New York single-state LPN license; NY is not a compact state.
• Possess at least 1 year of relevant LTC/SNF experience.
• BLS certification is required.
• Demonstrate EPIC charting experience.
• Be able to float within skillset and complete modules as required.
• Reside within 75 miles of the facility (preferred, but not mandatory for the position).
