• Administer medications and treatments according to physician orders
• Monitor and document resident conditions, changes, and responses to treatment
• Provide direct nursing care and support to residents
• Collaborate with RNs, physicians, and interdisciplinary team members
• Maintain accurate and timely patient records
• Ensure compliance with facility policies and state regulations
• Promote a safe and comfortable environment for residents and families

• Current New York State LPN License
• Minimum 1 year of experience in a Skilled Nursing Facility (SNF), Long-Term Care (LTC), rehabilitation, or similar healthcare setting
• Strong clinical assessment and communication skills
• Ability to work independently and as part of a team
