Senior UAS LTSS Assessor; RN

The Senior UAS LTSS Assessor (RN) performs care management duties to assess and coordinate medical services for complex populations, developing personalized care plans and educating members and their families on available services.

• Evaluate service needs of complex or high-risk members and recommend plans for optimal outcomes.
• Develop and assess ongoing long-term care plans in collaboration with the care management team.
• Coordinate care between members, families, and providers to ensure person-centered services.
• Monitor care plans and member status, revising as necessary to meet goals.
• Review member data to identify trends and improve care quality.
• Collaborate with healthcare providers to facilitate member services and treatments.
• Collect and maintain member information to ensure compliance with guidelines.
• Educate members and families on disease processes and healthcare options.
• Act as a liaison between members, families, and healthcare providers.
• Provide coordination of service authorization based on assessments.

• Graduate from an Accredited School of Nursing or Bachelor's degree preferred.

• 4–6 years of related experience.

• RN
  - Registered Nurse
  - State Licensure and/or Compact State Licensure required, OR
• NP
  - Nurse Practitioner
  - Current State's Nurse Licensure required.
