About This Role

The Nurse in Palliative Care at Home provides home-based palliative symptom assessment, treatment, and ongoing goals of care discussions for patients and families facing advanced illness. This role is ideal for nurses who are passionate about delivering high-quality, evidence-based complex palliative care in a home setting.

What You’ll Do
  • Work as a member of an interdisciplinary team assessing and addressing the complex care needs of patients and families facing serious illness.
  • Visit patients and families in their homes to complete guided assessments, support medical management, address complex symptoms, and coordinate care.
  • Provide supportive individual and family counseling to reduce anxiety/stress and symptoms precipitated by advanced illness and hospitalizations.
  • Engage patients and caregivers about goals of care, review advance directives, and collaboratively develop treatment plans based on goals.
  • Monitor and coordinate care plans in conjunction with the rest of the clinical care team.
  • Evaluate medical needs for a caseload of patients and provide them direct in-home care.
  • Facilitate visits by the providers remotely as needed.
  • Communicate the plan of care to consulting clinicians and PCaH interdisciplinary teams.
  • Educate patients/families on the trajectory of illness, use of medications, and assess disease understanding.
  • Participate in weekly IDT meetings with the care team.
Requirements
Education
  • Bachelor’s Degree in Nursing (BSN) required;
    Associate’s Degree in Nursing (ADN) acceptable if matriculated in a BSN program with a graduation date within the year.
Experience
  • 2+ years of experience managing patients with medically complex and chronic/severe illness.
  • Experience working in interdisciplinary care teams.
Licenses & Certifications
  • Licensed as a registered nurse with current registration in New York State.
  • Basic Life Saver (BCLS) certification required.
