A well-established skilled nursing facility (SNF) in Brooklyn is hiring an experienced Lead MDS Coordinator (RN). This is a full-time Lead MDS Coordinator role paying up to $165,000 a year, and we are looking for someone who has done this job before. Prior MDS Coordinator experience in a nursing home / skilled nursing setting is required.
As Lead MDS Coordinator, you will own the Minimum Data Set (MDS) process for the building and guide the MDS team, not just complete your own assessments. If you are a Registered Nurse (RN) who knows the Resident Assessment Instrument (RAI) process inside and out, understands Patient Driven Payment Model (PDPM) reimbursement, and can keep a full assessment schedule accurate without hand-holding, this Lead MDS Coordinator position pays well for what you already know.
The Lead MDS Coordinator works directly with the Director of Nursing (DON), therapy, and the interdisciplinary team, and is treated as a key part of clinical leadership.
What You'll Do
- Lead and support the MDS team, setting priorities and making sure every assessment is completed and submitted on time
- Review and audit Minimum Data Set (MDS) assessments for accuracy before submission, following the Resident Assessment Instrument (RAI) manual
- Own the assessment calendar and track Assessment Reference Dates (ARDs) across the building
- Keep Patient Driven Payment Model (PDPM) reimbursement accurate and catch missed opportunities in documentation
- Train and mentor MDS staff and floor nurses on documentation that supports accurate assessments
- Coordinate with nursing, therapy, social services, and dietary so care plans reflect each resident's assessment
- Support the team on quality measures and survey readiness
Qualifications
- Active Registered Nurse (RN) license in New York State
- Prior experience as an MDS Coordinator in a skilled nursing facility (SNF) / long-term care (LTC) setting is required
- Lead MDS Coordinator or multi-coordinator team experience strongly preferred
- Strong working knowledge of the Minimum Data Set (MDS), the Resident Assessment Instrument (RAI), and the Patient Driven Payment Model (PDPM)
