Dedicated LPN: Lead Resident Care & Team Coordination

Optima Care Partnership is seeking Licensed Practical Nurses (LPN) to provide and coordinate resident care at Little Neck Care Center in New York. The role includes direct patient care, education for residents and families, and collaboration with the interdiisciplinary team to implement individualized plans of care.

Responsibilities include charting, medication administration as ordered, and supervising non-licensed staff.
